Proof of Life

Authorities may request written proof that you are still alive. There are two types: a proof of life or an attestatie de vita. You can request these documents from the local government where you live or are staying.

What does this product or service mean?

A proof of life and an attestatie de vita are written proofs that you are still alive. Authorities such as a pension fund or benefits agency may request these documents.

The difference between the two documents is:

  • A proof of life is used within the Kingdom. This is an extract from the Civil Registry. The application does not need to be submitted in person.
  • An attestatie de vita is used abroad. This is a certificate issued by the civil registry. It is an international document. The application must be submitted in person.

When requesting an attestatie de vita, you can also request a multilingual standard form at the same time.

When can you apply for this?

To obtain a proof of life, you must be registered in the Civil Registry.

To obtain an attestatie de vita, you must visit the Civil Registry office in person.

How do you apply for this?

Here’s how to request a proof of life:

  • Visit the Civil Affairs Department in person or send a representative to the Civil Affairs Department
  • In person? Please bring:
    • your valid ID
    • if applicable, the letter from the agency stating that you need a certificate of life
  • You must pay the fee when you submit your application. Please pay by debit card.
  • Authorized representative? Please provide:
    • a copy of your valid ID; the authorized representative must also present their own valid ID
    • the written authorization
    • if applicable, the letter from the agency stating that you need a certificate of life

  • Go in person to the Civil Affairs Department
  • Bring the following:
    • your valid ID

Is your pension fund requesting the extract? If so, it is free of charge. Bring the statement from the pension fund as proof to receive the extract for free.
